Can high doses of Rosuvastatin harm the kidneys?


To investigate, Jung-Im Shin, MD, PhD (Johns Hopkins Bloomberg School of Public Health) and her colleagues analyzed electronic health record data of 152,101 new users of rosuvastatin and 795,799 new users of rosuvastatin. another new statin called atorvastatin from 2011 to 2019.

During a mean follow-up of 3.1 years, the team identified hematuria in 2.9% of patients and proteinuria in 1.0% of patients. Compared with atorvastatin, rosuvastatin was associated with an 8% higher risk of hematuria, a 17% higher risk of proteinuria, and a 15% higher risk of developing renal failure requiring replacement therapy such as dialysis or transplantation.

The risk of hematuria and proteinuria is higher with higher doses of rosuvastatin. Additionally, among patients with advanced kidney disease, 44% were prescribed doses of rosuvastatin higher than those recommended by the US Food and Drug Administration for those with poor kidney function.

“We observed a higher risk of hematuria, proteinuria, and renal failure with rosuvastatin and similar cardiovascular benefits between the rosuvastatin and atorvastatin groups,” said Dr. “Because rosuvastatin can cause proteinuria and hematuria, especially at high doses, high doses of rosuvastatin may not be worth the risk – even if small – especially for patients with advanced kidney disease. .”
